Dear Parent or Guardian:

The Wilma P. Mankiller Health Center is unable to come to our school or any other Adair County Schools to complete physicals for students in the 5th-8th grades that plan on participating in athletics for the upcoming school year. However, they will be making appointments for physicals over the summer, beginning the first Friday in June, so parents of students that can utilize the Wilma P. Mankiller Health Center can begin calling the clinic now at (918) 696-8800 to schedule their child’s physical. Students that are not patients of the WPMHC will need to have their parents schedule their physical appointments with their Primary Care Physicians. The School will make physical forms available in the Elementary and Jr. High Offices, if you or your child would like to pick one up. Forms will also be available at the clinics and doctor’s offices.

Please keep in mind, that your child’s health and well-being is our foremost concern and physicals help ensure that your child is safe to participate in sports, so students that do not have a cleared physical form turned in to the school nurse at the start of the school year will not be eligible to participate in athletics.

We appreciate all your cooperation in acquiring your child’s physical.

Thank you,

Nurse Leach
